Carotid pulse: Often you can hear your carotid pulse especially when lying on your side. Usually not a concern!

Pulsatile tinnitus : Pulsating noise can be caused by wax impact ion, ear fluid, high riding jugular bulb in the middle ear, vascular tumors, hypertension, carotid stenosis, intracranial hypertension, and vascular anomalies. Does neck compression change the sound? Go see an ENT or otologist.
