See below : Your description suggests the presence of inflammation/infection involving foreskin and glans penis (aka balanoposthitis). Most common cause: fungal (yeast) infection, especially if diabetic, and usually responds to good penile hygiene, and glucose control IF diabetic, and topical or oral antifungal Rx. Also, a variety of dermatological lesions can cause similar findings. Recommend urology eval.
