The "SPACE": the top of the shoulder needs to move freely is too small since the bursitis and the bony spur BOTH IMPINGE on that space. This is called "Impingement Syndrome" Steroid injections might give some relief but the ultimate treatment is surgery (Called "Subacromial Decompression" Hope this is helpful DrZ
