Eustachian tube: When you swallow a muscle within your neck straightens the tube that allows air to enter the middle ear. If your Eustachian tube is partially closed, that event can allow a rush of air in that changes the contour of your eardrum & causes pain. There are other possibilities but this sounds like one. Your doc can confirm/exclude this & see if treatment is needed.
