Mechanical issue: Reflux is a backwash of stomach contents. It results when the circular muscle closing the swallow tube is unable to resist the pressure made by the stomach muscle as it churns/mixes the feed. It is aggravated by a full stomach, laying flat & tends to increase from birth, peaking at 3-4 mo & only reducing after they sit up at 6-7mo. Formulas don't matter. Thickening & small feeds may help.
