Yes: The cause of his high glucose should be determined right away. If he is taking insulin the dose will need to be adjusted and a follow up appointment with Endocrinology is suggested.

New issue ?: If you know his glucose levels he is likely diabetic & should have a diabetes team following him. My first effort would be to call his team, who should know his case well and follow their instructions on further care. If he is not followed by a diabetes team it is time he had one and a trip to the hospital to stabilize his current condition would be a good first step.
