Because doctors and patients often cannot know the cost of a visit until after a visit, because the government likes to have a hand in everything, and because insurers don't enjoy paying for services, there ends up being excessive rules. It is possible that SEPARATE billing for a SINGLE FOBT is not standard procedure.
