Call the doc.: If a medicine combo is not working, let the doc know so it can be adjusted. Consider a second opinion with another psychiatrist if that is not successful. Of course, if your friend/loved one wishes to hurt him/herself or others, or is hearing or seeing things that others don't hear or see, call 911 or take him/her to the ER. Hope your friend feels better soon!
