100.2 fever for 3 months in afternoons, fatigue, swollen lymph nodes & elevated lymph count, rbc and differentials normal. ana 1:320 speckled (t1d and post thyroid cancer) all other tests normal, metabolic panel, sed rate, crp, etc. what can it be?

Elevated ANA, LN,fev: https://fpnotebook.com/rheum/Lab/AnStngPtrn.htm Speckled pattern is suggestive of lupus as is the elevated ANA and lymphadenopathy, fever. Speckled can also be seen in Scleroderma/ Sjogrens mixed connective tissue. But with this picture you have to consider Castleman's disease, sarcoid , lymphoma, TB, burcellosis if cattle contact, syphilis and HIV. follow up with rheumatology/ ID
