Nipples cracking: This could be dermatitis, infection, fungal infection, or early Pagets of the breast. If there is any asymmetry or discharge please get examined as soon as possible. It would be prudent to get a good exam no matter what.

Nipples itch: Eczema of the nipples is a common cause of these symptoms. Other possibilities include reaction to bra material or simple irritation and friction. A cortisone type ointment should help if eczema is the cause.
