"5 wks. pregnant" ?: At five weeks of intrauterine pregnancy, an ultrasound should show findings consistent with an intrauterine pregnancy. Get a serum HCG test to confirm pregnancy, and if positive, get a repeat ultrasound in two weeks. If bleeding starts, see your OB provider to make sure the pregnancy is not outside the uterus ("ectopic").
