Otitis-externa: If you are experiencing pain when pulling the outside of your ear, you likely have an infection/irritation in the ear canal (this is also called "swimmers ear"). The treatment is usually topical (drops), but its necessary for a physician to directly visualize the problem as the canal may need to be cleaned out in the office, and the ear drum should be inspected as well to make sure its intact.
