Hydronephrosis: If the hydronephrosis was found 20 years ago, then the left kidney function could be very minimal. Nothing can be done in that case. But if you are 20 years old and recently found the hydronephrosis, then that is an emergency visit with urologist ASAP to decompress the kidney and find the cause of the hydronephrosis.

Wait and see....: Apparently, you have been lucky with no symptoms from the incidentally found left hydronephrosis over the past 20 years. Clinically, doing nothing with a watch and see would be the most reasonable thing to do after a set of evaluation with history+urinalysis+blood test for kidney function and renal US as base line info. Meanwhile, avoid vigorous contact sports so to minimize injury to such kidney.
