You have chronic: Constipation. It is normal to have , at least , one BM per day. The longer the stool stays in the colon the more water is taken out of it, resulting in a very hard and large stool. This is very difficult to pass. You should seek help with this by visiting your primary care physician. They may decide to refer you to the specialist , a Gastroenterologist, for a more complete work-up.

Constipated?AnalScar: Since you said once a week, I assume you refer to painful bowel movement. Constipation with hard stool is most likely cause. Treat with sort term use stool softener while diet alterations to include more fiber. Hemorrhoid problems with inflammation or thrombosis is common cause. Anal stricture- tight scarred anus - less likely. Both issues need attention by doctor. See a doctor for better eval.

Constipation: Increase the fiber in your diet and try an over the counter stool softener. See your doctor if there is no improvement.
