How can i tell the difference between pleurisy pain and heart spasms or heart attack?

Pattern of pain: It may be easier to differentiate the pleuritic pain from heart attack pain. However, it is quite difficult to differentiate between heart attack and heart spasms. Typical pleuritic pain is described as sharp pain that worsen with breathing. Commonly seen in pneumonia, viral infection, etc. Typical heart attack pain is described as pressure liked, "elephant sitting on the chest" type pain.
