Right heart check: People with severe asthma and people with potential sleep apnea (sleep apnea also can be present in with severe asthma), the blood pressure in the vessels of the lungs can get high. This eventually can strain the right side of the heart and cause fluid build up in the ankles. Your doctors are checking on the status of your right side of the heart by checking your ankles. Hope this helps.
