Multiloculated: A pleural effusion means there is fluid in that chest cavity. Multiloculated means that the fluid isn't just one single continuous collection but is either multiple or divided in some fashion. With high fever, rib pain and back pain infection is certainly a concern and you need to seek care urgently.
