Don't know: If your doctor stated that you may want to ask for more information. I would need to know more effective than what and for what (pain, mood, less side effects?). Was this based on a validated double-blinded study on end-stage cancer patients, and so on. If needed a combination of widely available opioids and non-opioid analgesics can be very effective for refractory pain.

Opioid strengths: I would recommend against this because 1. Heroin is illegal in this country 2. Heroin acts for only a very short time, and would have to be dosed very often. You can't help your loved one if you are in jail. Stick with long acting legal painkillers such as sustained release morphine or dolophin. Better yet, consider non-opioid pain meds such as Gabapentin or duloxetine.
