Painful scalp lesion: There a few possible causes of this painful scalp lesion. If it's raised, like a lump, it is most likely due to a skin infection or lymph node trying to fight a local infection. If the area is flat, I would imagine you are describing muscle tension in a localized spot of the scalp. See your doctor to have a hands-on and visual inspection of the problem area. I hope the problem resolves quickly.
