Formal evaluation: It sounds like you need a formal evaluation including MRI of the brain. this been there 20 years it may be in the inner ear but nonetheless this is the best way to objectify the problem

Tinnitus: I looked on your profile, and did not see how old you were. In general, tinnitus or ringing in the ears is caused by a hearing loss. Most commonly, it is in the higher frequencies and occurs in older individuals who were exposed to loud noises in their work environment. You need to get a hearing test and see where your hearing is at this moment. Once you know this you can proceed to next step
