Using rectal glycerin or dulcolax suppositories may help evacuate better. Fleets enemas can be used if you need more help emptying rectal contents. A gastroenterologist or a proctologist may be helpful for you.

Could be serious: especially since you're diabetic. You need to be checked for several things, please see a doctor. Meanwhile drink plenty of liquids and eat lots of fiber and vegetables.
