"Structural" problem: The cystocele is a result of the bladder support tissues (structural) weakening or failing, and not due to vaginal thinning mucosal issues which estrogen treats. It is the equivalent of a house's foundation/walls weakening(cystocele) versus the need to replace a "threadbare carpet"( vaginal mucosa estrogen Rx).
