Records release: You need to execute a records release to any doctors or hospitals that you may have received care from in the past. An Operative report in those records will determine if the Fallopian tubes were in fact ligated or interrupted. If all else fails, a GYN MD can perform either a HSG dye study or a Laparoscopy that visualizes the Fallopian tubes for ligation.
