Dental problems : The apple may have upset some underlying gum or dental disease such as infection that was brewing. Recommend check with a dentist or doctor for a mouth examination

Pain: Could be tooth fracture, bone fracture, tooth displacement. Probably a good idea to see your Dentist for a baseline exam , determination of cause of the pain, and appropriate observation or treatment.
