? respiratory: infection, which can be viral, sounds like it is affecting both upper, nasal cavity and sinuses, and lower, lungs and airways, producing the symptoms noted. The shoulder blade pain can be muscular or a referred pain (pleuritic pain). Need to see your doctor as you may need Rx meds, meanwhile, rest, fluids, & pain killers, OTC cold meds may help
