It is what it is: Most folks with absolute iron deficiency have a high iron binding capacity, but I wouldn't bet my life on it. Some folks don't increase the transferrin as well as others do, from their own physiology or coexisting illness -- acute phase reaction famously keeps transferrin low. You need to be checked for abnormal blood loss, get a thorough physical exam, and consider iron Rx.
