I assume: you are referring to a THYROID GOITER or enlargement. RAI will not work for PARENCHYMAL disease since this tissue does not "employ" IODINE to make thyroid hormone ( Thyroid FOLLICULAR tissue does that "job") Hope this is helpful Dr Z
