In some areas: Meconium can pass ant any time before/during/of after labor/delivery. If I look at the umbilical cord, it will often show meconium staining that reflects passage prior to or during labor. The more intense the stain the longer the kid was surrounded by the material before delivery. Even small amounts may stain the fluid enough to stain the umbilical tissue. The midwife may never see it.
