Why is my methadone wearing off in 12 hours sometimes when i take 140 mg?

Methadone: Methadone is a long acting med. So your report of "wearing off" does not fit. Also your dose of 140mg/day is excessive and I would strongly suggest that you attempt a wean down. I assume this is not being used for opioid use disorder.
