The tendon is : Inflamed. It could be you have a small tear or irritation in the tendon. And the surrounding tissue called synovium of the tendon is swollen.

Incidental: It may be an incidental finding. The flexor hallucis longus is a tendon on the inside (medial) aspect of the ankle and onto the sole of the foot, it flexes the big toe. Tenosynovitis basically means it is inflamed, but again, that would cause medial and not lateral ankle pain. I would discuss with your physician and look at the MRI findings on lateral ankle. If normal, may be from the spine.
