I recommend: medical assessment for two reasons. One is to find out if there is a medical problem that contributed to you passing out - the second is to assess for injury.

Issues are many...: Passing out and falling to the ground at age 46 years can be a serious health warning. Has one been drinking or medicating heavily? Hypertension? Chest Pain?, heart Disease? Dizziness? Seizures? Diabetes? Hungry? Dehydration?.... The issues are many. Best to see PCP to evaluate status & prevent future occurrence. Chest hitting the car side mirror is also an issue to be evaluated as well.
