Back pain : Due to long duration of this pain, worsening of symptoms, and extension down to toes, you should see a Neurologist for further evaluation. Once you have the right diagnosis, you can get the right treatment.

See your doctor.: Low back pain can be conservatively managed with rest and relaxation initially, but once you begin feeling pain going down to your toes, it is important to get checked out to be sure that you're OK. I would make an appointment with your doctor for a physical and further work up if necessary.
