I took my 1 yr daughter into the er for a fever and throwing up they checked her temp by rectal thermometer. they inserted it almost 3 inches in her! she screamed and threw up immediately after!is there by chance they could have seriously hurt her?

Doubt it: The rectum is a tube with thick walls. You can insert a tube up to 6 inches or so before it must turn a corner in a 1yo. I have never seen any 1 yo thrilled to have a rectal temp or seen one with an injury from such a procedure.
