I have cirrhosis and am on the transplant list. liver team told me no more than 2grams tyenol/24hrs. orthopaedic who is treating torn rotor cuff suggested advil (ibuprofen) vs tyenol. kidney metabolizes advil (ibuprofen)?

Ibuprofen: Tylenol (acetaminophen) is broken down in the liver, which in your case isn't recommended. Advil is ibuprofen, which is broken down in the kidney. It is an anti-inflammatory and would help with your shoulder.

Tylenol (acetaminophen) vs Advil: Tylenol (acetaminophen) (t) is metabolized by the liver and advil (a) is not, so a can be used for pain relief without harming a liver that is already diseased. A does have some side effects, such as causing stomach problems as well a rare cases of renal failure, which are reversible once a is stopped. Speak to a gastroenterologist about your need for pain relief, rather than an orthopedist for your answer.
