Interstitial tear.: Insertional interstitial tears of the subscapularis tendon have recently been described in the medical literature. They are a cause of chronic shoulder pain & aren't usually seen on arthroscopy unless the arm is moved during the arthroscopy (when the arm is rotated internally, the articular side of the tendon bulges like an air bag).
