Carpal tunnel?: I think before you try for any sort of note (which you couldn't get from us anyway), you should be evaluated by a Hand Surgeon for consideration of carpal tunnel syndrome. This can be corrected, which means you'd be able to continue working comfortably.

Tendenitis?: This may be from inflammation at the elbow . If numbness in fingers is also present, the result of nerve entrapment. Often a change in the sitting position and holding the mouse further away from your body may help. An orthopedic consult appears to be in order if you don't want to quite this job.
