Positional chest pai: Greeting, The differential for chest pain is wide and some possibilities are dangerous and an updated cardiac exam is recommended, however positional chest pain tends to be pericarditis,musculoskeletal,or reflux. Gastric reflux causes pain when we lie back and to the side and gravity facilitates acid refluxing into the esophagus causing esophageal spasms that can be quite painful

Chest pain: Did you have these symptoms last month and then have the cardiology workup? Did they do EKG and stress test? Does it come with exertion, stress? If this is new symptom for you, I would have it evaluated.

Normal heart test?: At your age, a sprained rib cage (costochrondritis) is the most likely issue. However if your heart test consisted only of an EKG , it may not rule out a coronary problem especially if you have risk factors (family history of heart attacks, high lipid levels, diabetes, hypertension etc) In case of doubt, consult a heart doctor just to be sure.
