Mechanism is damage to the brain's vestibulo-ocular reflex. Seen after meningitis, multiple cranial neuropathies sec to carcinomatous meningitis, severe head trauma, but can be seen with stroke, MS, localized tumors, even Meniere's. Transient nystagmus from toxic doses of medication can mimic this
