See your GYN: Sounds like you need a follow up appointment with your GYN doctor for a possible ultrasound. If you have sudden onset of severe sharp pain in the area you should go to the Emergency Room as a twisted ovary is possible and that is a surgical emergency.

Sonogram+Gyn consult: If your 3 year old ovarian cyst is causing pain, ask a credentialed and skilled sonographer to perform a careful evaluation of the sonographic features of the ovarian cyst (sizes, volume, echogenicity, wall thickness etc...) and of its blood flow by color Doppler and seek an experienced Gyn to advise you about the risks/benefits of measuring your "tumor markers" and of a laparoscopic operation.
