UPJ stenosis: Ultrasound may detect early obstruction better than ct, and sometime the other way around. It depends. Anyway, left flank pain with mild hydronephrosis, and no stone is usually due to a narrowing of the joint between the ureter (pipe to the bladder) and the renal pelvis (holding area for urine). If there is a mild or partial "blockage" that could explain everything.
