Thrombosed Pile: Some piles might get the blood inside clotted and causes swelling and pain You might need it to be checked out if it continues to hurt as it might need to be cut open and cleaned.

Likely hemorrhoid: Typically piles are not painful. More commonly hemorrhoids are cylindrical and can become painful when thrombosed. A thrombosed hemorrhoid will become firm and painful to touch. You should see your doctor and have an exam.
