Could be a cyst: It could be a cyst or an appendage Being there for years and not changing in size or causing trouble it highly likely to be benign in origin and not to worry about it.

Sounds like: a possible epididymal cyst; however., due to your age I suggest a urology consultation and ultrasound of the testcles and scrotum
