Both: If you are looking for pain relief, either works fine. If you want some relief of inflammation associated with the sprain/strain, Ibuprofen works better. However, in the first 24 hours after the injury when there may be active bleeding (oozing of tiny blood vessels in the injured muscle /tissue), using Ibuprofen could ad just a tiny bit to the bleeding because it is a blood thinner. So ice & rest!

Tylenol (acetaminophen): They both will treat the pain with similar efficacy. Advil (ibuprofen) will last a little longer and costs a little more but also has slightly more side effects. I'd go with tylenol, (acetaminophen--generic is fine) and just repeat it every 4 hours. If it doesn't go away after a few days or gets red and hot, get to your physician.

Advil (ibuprofen): Advil (ibuprofen) is a better anti-inflammatory. Do not take on a empty stomach.
