I am contacting my surgeon, but i am wondering how much impact is needed to cause issue with 5 month p/o l5s1 fusion. i fell on ice, pain increased.

Rarely an issue: If you think about the back like a chain, the fused segment is by far the "strongest link" in your spine. As such, if you were to injure your back in a fall, it would be much more likely that you would injure an adjacent level than to hurt the fused area itself.
