Not necessarily: Majority of the cases of disc herniation settle by itself after a period of time and does not need surgery It depends on how severe your condition is as per the scan. A surgeon who checks you out and check the scan is the one to decide if your condition needs operation or not.

Foraminal indentatio: Would not seek surgery unless having EMG/ NCV evidence of nerve involvement, weakness/numbness , nerve(not fat )compression and severe pain. Back surgery is complex and outcome can be variable.. Lots of individuals have abnormalities and have no pain, and there are those with pain and minimal changes. So MRI alone is insufficient to assess need for surgery. Get 2nd opinion
