Water in cheeks: "Water" retention is always due to salt retention. Son look at your salt intake first.

Puffy cheeks: Metformin may help in PCOS. Aldactone which is a type of diuretic (water pill) may also help. Be mindful of the fact that "water retention on cheeks" may be due to other causes other than PCOS. Have you been evaluated for cushings?
