Frostbite : This thermal injury sounds like frostbite to me. I think you need an urgent evaluation at a burn center. This sounds like third or possibly even fourth degree frostbite. You also need to update your tetanus vaccination and possibly be placed in a hot whirlpool, get debridement of dead tissue and education on dressing changes and follow up with a burn surgeon. This is Advanced Trauma Life Support

Rest, ice, NSAIDs: Assuming the diagnosis of bursitis is correct, initial treatment includes rest from aggravating activities (particularly overhead activities and lifting), ice to the shoulder for 20 minutes 3-4 times/day and anti-inflammatories (nsaids) such as advil or aleve (naproxen). Make sure that you read the warning label before taking these. Do realize that subacromial bursitis often occurs with a rotator cuff tear.

Insect bites: You are describing the normal healing of a bug bite that gets inflamed and then gradually heals. At this point, if there is any increase in redness, pain or swelling, then additional treatment with antibiotics may be needed. Warm compresses on the skin can help.
