Vascular: Thyroid nodules can be very vascular and may not have a firm supporting structure so vessels in the nodule can break causing bleeding into it. Sometimes it can lose the blood supply in the center and become necrotic and then hemorrhage. Rarely there can be a viral thyroid infection causing swelling and pain.
