Could be a boil: If the lesion is on the skin and not attached to underneath, it might be a skin boil or skin infection (given it is painful) I would suggest to get it checked by your doctor to confirm diagnosis and to arrange the right treatment for it

See below : Most common cause of a lump within the scrotal skin is a sebaceous (aka epidermal) cyst. Caused by plugging of the sebaceous glands. Leads to a smooth, round, firm, off-white in color lump. Usually painless unless irritated or infected which commonly occurs when one tries to squeeze and "pop" it. Avoid this as it is not like a "pimple." Treatment: if large or bothersome, surgical excision.
