The difference : Is the location of the injection. The transforaminal space is adjacent to the dura and epidural space. These injections are more site specific than epidurals.

Transforaminal Inj: In the transforaminal approach to the epidural steroid injection the the epidural space and nerve(s) is coated with the steroid/anesthetic mixture. The epidural injection you refer to is called an interlaminar or translaminar epidural injection where the medication is placed between the two spine bones in your back. They are: both epidurals, i.e., needle placed outside the dura. With the transforaminal the needle enters the epidural space from the side and requires x-ray guidance. With the posterior approach that is commonly used for labor and delivery and also pain management the needle is placed by sensing when it enters the epidural space between the vertebral bones in your back
